Notes
1. This form needs to be filled out by the registrant when he or she will start employment.
2. If the employment is on a trial or temporary basis, this declaration still has to be filled and the person should stop registering for work.
3. This declaration should always be filled on the first day of employment or the work trial.
4. Should you require copies of this form, these can be obtained from the District Offices of the Department of Social Security, Jobsplus Job Centre or Jobsplus website www.jobsplus.gov.mt.
5. This declaration is not the Engagement Form. Therefore, if you are starting a new job, whether you will be working as self-employed or an employee, you will always have to fill in the Engagement Form and send it to Jobsplus.
